Parental Agreement to Administer Prescription or Non-prescription Medicine

From time to time children will require prescribed medicines to be administered in school. Attached is a form to complete which will enable staff members to do so on behalf of the parent/carer.  Without this completed form we are unable to give a child medication.  When necessary and wherever possible to reduce paperwork into school, please complete and email the form to the school office on admin.galmpton@acexcellence.co.uk.  Medicines should then be handed to the staff member on entry to school for safe keeping.
